Linking Risk Assessments to a Care Plan

1. Navigate to Care > Care Plan Detailed.




2. Click Options on the right side of the screen and select Change Student.




3. Use the filters to find the Student.


When you have found and selected the student, click Close.




4. Select one of the Care Plan Categories.




5. Click a Statement, in this case I will use My Hobbies And Interests Include.




6. Select the Risks tab.




7. Click Add Risk.




8. A Risk Assessment form will appear, fill in all of the fields.


The Assessment Type & Hazard/Risk dropdowns link the students risks to this section of the care plan.




9. Here is an example of a completely filled out form.


Click Save when complete.




10. The Risk Assessment will now appear with a Score in the Risks tab.


Select Save.




11. To edit the Risk Assessment, click the Notepad icon.
