In this article you will be shown how to create a Care Plan template on Databridge, that will allow you to print a student's Care Plan on to your chosen document.
1. Care Plan Templates are used to print Care information to a custom template that you have created. To do so, navigate to Care > Care Plan Templates.

2. From this screen you will see any existing templates in the left hand column of the page. In the column on the right will be an index displaying all standard bookmarks.

3. You will first need to enure that your desired template has been created on a word document.
Include Key Information:
- Add placeholders for details such as the student’s Name, Age, and Contact Information (you can bookmark this information from the students record).
- Insert any necessary images.
- You can also create bookmarks for any Statements and Questions.
Bookmarks can be created from a standard list, as seen in step 2, or custom bookmarks can be created to pull information directly from a Care Statement's textbox.
- Standardised Bookmarks follow the formula of [db:Example] where the word Example is replaced by a bookmark from the list as shown in Step 2.
- Custom Bookmarks use the formula [db:ST_##] where the Hash Signs are replaced by the Care Statement number which can be found via Administration > Standard Values > Care Statements. The number is shown as the end of each statement as per the screenshot below.

- For any questions within a care statement, you would use [db:QT_xxx] where xxx is the ID of the question.

Here is a simplified example on a word document using default and custom statements (before uploading the document and assigning to a student).

4. When the word document is complete, select Add New.

5. Enter the care plan name into the Title textbox.
Choose File and upload the word document you created.
Click Save.

6. This template will now appear in the Templates section.
You can click the Downward Arrow icon to download the template.
Clicking the Trashcan icon will delete the template.
