Creating a Care Plan on Databridge

1.  There are a couple of options when it comes to adding a Care Plan Template to a student. The quickest way to do so, is to navigate to Care > Care Plan Detailed


2. Clicking Care Plan Detailed will load the page as so. To select a student, navigate to Options > Change Student (You can use this to move between students from the same page). 



2.1. Clicking Change Student will load a window, where using the filter options you can select the student you wish to access via the Student dropdown. Once you have chosen student, click Close.



3. Clicking Close will take you to the students Care Plan. To start adding the previously created template, click Add Statement(s). 



4. Clicking Add Statement(s) will load a window with all the statements on your Databridge (setup within Standard Values). 



5. Clicking the arrow next to a Statement will show all the attached subcategories and their statements. 



5.1. Clicking the blue + symbol by a statement will select it to be added to the students Care Plan. 



5.2. If you know that you wish to add a whole category of statements, in this example, Residential, you can click the List symbol, and all the statements will be moved into the added section. 



5.3. If you wish to add a custom statement, clicking the symbol will open a window where you can add the Custom Statement and any Notes before clicking Save



6. Once you have selected the statements you wish to add, click the green Save button. 



7. Clicking Save will take you back to the students Care Plan. The category you selected will now appear, and clicking the statement will open the category showing all the statements attached. 



8. Clicking one of the statements will open a window where you can populate the data for that student’s information. 





9.  It is worth noting that you can also access a Student’s Care Plan by navigating to Student (this is a customisable label on Databridge and as so, this may be labelled differently on your Databridge) > Find Student > Select the relevant Student from the list > View Student



9.1. Clicking View Student will load the students Profile. From the Students Profile, navigate to Care Plan in the navigation menu on the right.
