Accessing the Care Plan Templates Screen
To access the Care Plan Template page, select Care in the module list on the left, and then select Care Plan Templates.
On the Care Plan template screen you will see all already existing templates in the left hand column of the page. In the column on the right will be an index displaying all standard bookmarks.
Creating a Care Plan Template
To create the template that you want to upload you'll need to use a Word document. Populate the word document as required, using the relevant bookmarks to specify where you want information to appear. To input a bookmark into your Word document, you will need to use this formula:
[db:Example]
Below is an example of what this might look like in a Word document:
This info is about [db:Student]
This person is [db:StudentAge] years old
They are from [db:StudentTown]
Their telephone number is [db:StudentTelephone]
When this template is printed, the result will be as follows:
This info is about Homer Simpson
This person is 27 years old
They are from Springfield
Their telephone number is 0987654321
using the information from the relevant learner record to populate the template where there are bookmarks.
Assigning Statements and Questions as Bookmarks
You can also apply bookmarks that will carry across the Default Notes section of a Care Statement itself. To do this use the following bookmark formula:
[db:ST_###]
using the numerical value of the Care Statement. See below for an example:
Here is an example of a Care Statement with its assigned number. The bookmark for this Statement would appear as:
[db:ST_244]
and when printed, will show the details of that bookmarked Care Statement.
For Questions you would use [db:QT_xxx] where xxx is the ID of the question.
Uploading your Care Plan Template
Once you have created your template on a Word document, you need to upload it onto Databridge. To do this, click on the Add New option on the right hand side of the screen.
From the window that appears, input a title for your template and then click Choose file. Select the relevant Word document, and then click the blue Save button.
Once saved your Care Plan Template will appear in the list of templates in the left hand column.
